Instant-access psychological therapy over the telephone

 

Telephone Psychological Therapy (TPT) is a unique new concept in the delivery of instant-access evidence-based therapy to anyone with a telephone. It is ideal for patients whose access to appropriate therapies through IAPT and secondary care services might otherwise be delayed or unavailable.

TPT offers personalised, portable, guided self-help psychological therapy modules that can be used at any time or place, literally at the touch of a button. It is a CBT based, stepped care intervention compatible with NICE guidance.
 

Choice of treatment modules for different presentations

A range of TPT modules is available for the treatment of different mild to moderate anxiety-based mental health problems such as generalised anxiety disorder, OCD and agoraphobia. Complements the NICE Stepped Care Model

TPT provides a therapeutic resource which fits neatly between steps 1 and 2 of the Stepped Care Model. It is suitable for patients who might not meet the severity criteria for referral to IAPT or secondary care services but who would benefit from timely intervention. Its use as part of a secondary prevention strategy can reduce the need for prescription drugs and remove the need for many patients to wait for treatment.

Fits patient needs and timetable

TPT is ideal for individuals who find it difficult to attend therapy sessions or who are keen to pursue therapy outside office hours and who need motivation and support. The 'therapist in your ear' needs no appointment, does not limit your access or penalise you for cancellations, is confidential, convenient and non-stigmatising. TPT saves time and money for the patient by eliminating the need to travel to appointments or take time off work to attend.

Relieves problems of access

TPT can be particularly useful in overcoming the problems of access faced by some patients because of: their responsibilities as carers of children, older adults or disabled relatives; their own physical disabilities; availability and cost of transport; gender or cultural difficulties with attending conventional therapy sessions; difficulty getting time off work.

Carers' support

Conventional means of service delivery potentially exclude the involvement of patients' friends and family members from the therapeutic process. With TPT, patients and carers can access the same psycho-educational material to facilitate a shared understanding of the presenting problems as well as the most effective way of supporting recovery and relapse prevention within the patient's social system.

Breaks through language barriers

A powerful feature of TPT is that it can also be made available in languages other than English. The need to provide mental health services to a growing population whose first language is not English is an increasing challenge for the NHS. Translating conventional one-to-one therapies or finding clinicians with the right language skills is time-consuming and costly whereas TPT modules can be translated once and used by many.

No need to read

As well as being easily made available in any language, TPT is also ideally suited to patients with poor eyesight or literacy skills which restrict their access to bibliotherapy or internet therapy. A version of the simple TPT user leaflet in large type is available for printing by the prescriber in his/her consulting room.

Easy to prescribe, easy to use

TPT is available 'on prescription' to patients by their GP or other healthcare provider. The therapy can then be regularly accessed by the patient making a simple free telephone call. An interactive, intelligent, computer controlled telephone system using voice recordings of therapeutic scripts and questionnaires will then guide the patient through a psycho-education, treatment and recovery programme that is responsive to the patient's individual presentation and circumstances.

Instant access

Psycho-education, guidance and structured self-help for managing their condition are available to patients wherever they are and at all times of the day and night. This immediacy of access is of particular benefit to patients with conditions that require help managing situational responses to emotional and environmental triggers or developing and maintaining impulse control strategies.

Advantages of using the telephone

The choice to offer TPT over the telephone rather than over the internet, on a PC or tablet computer or on a smartphone was dictated by considerations of service-user accessibility and the demographics of available technology:

  • Treatment needs to be free at the point of use
    • Service-users make a landline call to an 0800 number or they make a mobile call to an 03 'geographic' number and then get a call-back from IVR system
    • CHL, not the client, pays for calls
  • 63% of adults do not own a smart-phone (OFCOM, 2011)
  • PCs are unsuitable
    • Difficult or impossible to use when out and about
    • 23% of the UK households do not have access to the internet and 8Mn 'have never used the internet'.

Getting access to TPT for your practice

The range of Telephone Psychological Therapy modules is available for purchase by NHS service commissioners to distribute to their prescribers.

  • Healthcare commissioners buy module licences - per CCG - giving prescribers access to TPT modules
  • Commissioners also buy a block of service-user licences for an appropriate number of patients
  • Each package comes with full support to set up and run the TPT service for the prescribing practice
  • Healthcare practitioners use a simple, secure website page to prescribe one service-user licence (a course of TPT) for use by a patient
